Admission Procedures
Memorial Hospital knows you’re nervous enough about being in the hospital, so we make every effort to streamline the registration and admissions process to get you to your room as quickly as possible. Your comfort and privacy are paramount to us.
Feel free to get a head start by filling out this form before you arrive. Consent for Treatment & Payment
When you arrive at the hospital, go to the main admissions lobby at the Plum Street entrance. (Download the Memorial Hospital Map.) Then, come to the main registration desk. Once there, one of our highly trained professionals will ask you a few questions and help you complete the necessary paperwork in strictest confidence. It’s generally a good idea to bring your insurance cards with you. Once your registration is complete you will be given an identification bracelet and escorted personally to your room. If you need to know more, please don’t hesitate to call.
Patient Access
Call the main hospital number (937) 644-6115 or (800) 686-4677 and ask for Admissions.
Surgery Admission
Since surgery patients may face different circumstances, an Admissions Case Manager will coordinate all paperwork and pre-admission testing for you to make things easier and faster. Your family members will be shown to a comfortable waiting area with complimentary beverages while you are in surgery. Afterward, your surgeon will meet with them in this area to discuss the outcome of your procedure. If you or your family members have any questions before your surgery call:
